Covid-19 Information & Wellness Questionnaire

To minimize the spread of Covid-19 we ask that you take a minute to read the following guideline information.


**Reception area will be closed indefinitely**


Please come to your appointment alone. Guests will not be permitted to wait in the spa. We will ask your guest to please wait in their vehicle.


Please arrive on time. Consider traffic when scheduling your appointment. If you are running late, we will NOT be able to provide you your service due to the required time necessary for sanitation maintenance. We will strictly enforce our 24 hour cancellation policy and ask that you reschedule.


Appointments are required. Walk-in service will not be available.


Please be aware that our restrooms will not be available. If necessary, please use restrooms located in the main lobby of the building.


Refreshments will not be served.


Upon arrival please wait in your vehicle until a team member calls/texts when we are available to provide you with your service.


NO MASK, NO SERVICE, NO EXCEPTIONS!


Prior to entering we ask that you please wait at our entrance and we will open the door and perform the following steps:


Temperature will be taken with a contact-less thermometer


Client and employee hands will be sanitized prior to being escorted to the service area.


All belongings will need to be placed on the floor. No personal items will be allowed on tables or counters. Please no outside food or beverage. Keep this in mind while visiting. We suggest visiting with minimal.


We will offer contact-less check-out. A credit card/debit card will be kept on file. If you prefer to pay with cash, please have exact amount available.


Plexi-glass partitions have been installed at both our manicure and reception areas along with vinyl dividers between our manicure tables and pedicure chairs.


We have installed floor decals at each manicure table as a guide to maintain 6ft distancing.


All employees have received multiple sanitation certifications to provide you with the safest services.


Employees will wear a mask, gloves, prescription glasses, safety glasses and/or shield at all times while at the spa.



Please answer the following questions 24 hours prior to any scheduled appointment.

1. Have you tested positive for Covid-19 within the past 14 days?*
2. Do you have or have had any of the following symptoms within the past 14 days? Fever above 100.4, chills, shortness of breathe or difficulty breathing, cough, sore throat, loss of taste, nausea, vomitting or diarrhea?*
3. Have you been exposed to anyone who has tested positive for Covid-19 within the past 14 days?*
4. Have you traveled outside the New England area within the past 14 days? If yes, please reschedule your appointment or be prepared to present a negative Covid test.*
